I was actually thinking of making a posting on this for a while.
It’s definitely possible that Josh would eventually get more surgeries to fix his appearance when he stops being so guilty…
But also, I can see him getting used to his teeth the way they are (they are filed for basic function) and not wanting to get any more surgeries especially since teeth implants are risky and can cause dire side effects while Josh is already sick of anything medical.
